The parameters, in order and in detail:

chapsubtitle = English subtitle as it is in the fan translation. It's not official, but it's what's everywhere, so...
jpkana = Japanese kana/kanji for the subtitle
jptrans = Japanese translation (not the transliteration!) for the subtitle
image = Screenshot of a scene in the chapter, preferably in a notable/main location, like Duster in Osohe Castle for chapter 2
size = If the image needs to be resized a little. If not, though, then put in a caption that would appear as a tooltip
mainplay = Main playable character. Basically just the character you control.
mainparty = Characters that join you in battle. Note that this does not include supporting characters that you can't control in combat.
mainlocations = The main locations in the chapter, ones that the chapter's plot heavily involves
